Shiatsu and the Nervous System: Why Slow Pressure Feels Different

People sometimes ask what it actually feels like when a Shiatsu session works. The honest answer is hard to put into words, because it's usually nothing dramatic.
Breathing slows. The jaw softens. Muscles that were holding stop holding quite so hard. Attention drifts away from whatever was racing around in your head and lands, quietly, in the body. Some people fall asleep. Some don't, but tell me afterwards they felt unusually still.
It's tempting to file all of this under "vagus nerve activation" — the phrase shows up everywhere these days. I've never been comfortable with it. The research on touch and the nervous system is genuinely interesting, but it doesn't support the idea that every Shiatsu treatment flips a switch from sympathetic to parasympathetic. It's messier than that — and more interesting.
That messiness has shaped how I work more than any single study has. I tend to use slow pressure, sustained contact and comfortable movement, and give people time to respond. This article is an attempt to explain why.

The Autonomic Nervous System
The autonomic nervous system runs the things you don't consciously run: heart rate, blood pressure, digestion, the pattern of your breathing. Textbooks split it into sympathetic — mobilisation, getting things done — and parasympathetic — recovery, digestion, repair. Fight or flight, rest and digest.
The split is useful, but it's not a switch. Your heart rate shifts when you stand up. Your breathing changes when you concentrate. Your muscles brace before you even move. Touch that feels predictable and comfortable lands differently in the body than touch that's sudden or threatening.
Bodywork happens inside all of that — a system that's constantly recalibrating, not a machine with two settings.
Touch Is Never Just Pressure
Touch starts as mechanical stimulation, but what you feel as touch is built by your nervous system. The skin is packed with receptors for pressure, movement, vibration, stretch and temperature. Those signals travel up through the spinal cord and brain, where they meet context: where you are, what's happened before, how safe you feel right now.
So pressure is never just pressure. The same amount of force can feel completely different depending on how quickly it arrives, where it lands, whether you're watching it happen, and whether you trust the person doing it.
There's a big 2024 meta-analysis in Nature Human Behaviour — 137 randomised studies of touch interventions — showing measurable effects across a pile of physical and mental health outcomes. It covers all kinds of touch, not Shiatsu specifically, so I don't cite it as proof of any particular mechanism. But it backs up something I've always felt working: how you touch someone matters as much as how hard.
Why Slow Pressure Feels Different
Shiatsu uses sustained pressure rather than rubbing back and forth. You gradually sink weight through a palm, thumb or elbow and simply stay there. There's a real difference between pressure that arrives gradually and pressure that arrives all at once.
Slow pressure gives the person time to register what's happening. Time to breathe. It gives me time to feel whether the tissue is softening, resisting, or asking for less. And there's always time to stop.
The nervous system isn't a passive surface receiving touch — it's evaluating it the whole time. None of this proves slow pressure "resets" anything. But timing and quality change how touch is perceived, and perception is an active process.
A session is a whole sensory environment — light, temperature, rest and expectation all shape how the body responds.
What About the Vagus Nerve?
The vagus nerve is real and it matters. But "Shiatsu activates your vagus nerve" is a step further than the evidence goes.
During a session there are a dozen things happening at once — breathing slows, posture changes, attention narrows, the room is quiet, someone is with you, you're lying down, you feel safe. Any of those can shift parasympathetic activity. In the 2020 Scientific Reports study the authors couldn't isolate which part of the massage caused the HRV change — they flagged breathing, tactile contact, interpersonal attention, all of it.
I think of it less like pressing a button and more like creating conditions. A sensory environment where regulation can do what it does.
Breathing Is Half the Story
Breathing deserves its own mention. People often arrive breathing high in the chest. Half an hour later it's slower and deeper, usually without anyone having said a word about it.
The evidence on slow breathing is actually stronger than the evidence on massage. A 2022 meta-analysis of 223 studies in Neuroscience & Biobehavioral Reviews found that voluntary slow breathing raises vagally mediated HRV — during the breathing and afterwards.
Which raises the obvious question: did the pressure change anything, or did the breathing? Or the quiet? Or all of it tangled together? I'd say tangled together. That's why I don't reduce Shiatsu to one mechanism.
Work around the hara often lets the breath settle on its own — no instruction needed.
CT Fibres — the Fashionable Explanation
Then there are C-tactile fibres — CT fibres — the current favourite explanation for why gentle touch feels good. They're real: unmyelinated nerve fibres tuned to slow, gentle, emotionally meaningful touch. A 2023 review in Neuroscience & Biobehavioral Reviews is worth reading if you want the careful version.
But most of that research is about gentle stroking on hairy skin. Shiatsu is mostly sustained compression through clothing. Different receptors, different story. Saying Shiatsu works because it "stimulates CT fibres" is a stretch.
The genuinely useful idea from that literature is simpler: neuroscience now takes touch seriously as something with emotional and regulatory weight — not just pressure and location.
Where Attention Goes Matters
The part of Shiatsu that gets least attention is attention itself. Someone arrives running through emails and deadlines. Twenty minutes in, they're noticing the weight of their body, the movement of their breathing, the difference between the left and right sides.
That shift — interoceptive awareness, if you want the technical term — usually happens without anyone telling them to "be mindful". The steady rhythm of touch gives attention somewhere to land.
Why I Don't Fight Guarding
When something feels tight, the instinct is to push harder. Sometimes that's right. But more pressure isn't automatically better.
Muscle tone isn't just about muscle fibres. It's shaped by sensation, pain, anticipation, the memory of old injuries, and how the nervous system is running that day. If strong pressure makes someone brace and hold their breath, more force just produces more guarding.
So I meet resistance instead of fighting it. Pressure comes in gradually. I watch the breath. I notice when the tissue starts to soften under my hands. Often, movement does the work that pressure couldn't.
How Sotai Fits In
This is why Sotai sits so naturally next to Shiatsu. If turning one way is hard and the other way is easy, we start with the easy direction. Add a little resistance, move without strain, release, then re-test the restricted side.
You don't always need to attack the restriction head-on for movement to change. From a nervous-system view, the person gets to experience movement without the usual effort and guarding. In a session I move between still pressure, meridian work, joint movement and this kind of active participation, depending on the day.
Channels and Nerves — Different Maps
One more thing worth being clear about: Shiatsu doesn't come from neuroscience. It comes from East Asian medicine — Qi, Blood, Jing Luo, Xue Wei, Yin and Yang. Those aren't translations of nervous-system words. A meridian isn't a nerve. Qi isn't electricity. They're different maps.
I hold both. Channel theory gives me a way of thinking about connections through the whole body. Nervous-system physiology reminds me the person under my hands is constantly sensing, interpreting and responding. I don't try to force the two models to be identical.
Deep Doesn't Mean Forceful
Deep work doesn't have to be aggressive work. You can reach real depth by waiting: the contact settles, the breath changes, weight transfers, and the tissue underneath starts to yield on its own. Then the pressure goes deeper without ever having to be won.
The question stops being "how hard can I press?" and becomes "how much can this body comfortably receive right now?"
Regulation, Not Just Relaxation
I prefer "regulation" to "relaxation". Relaxation happens, sure. But so do other useful things: noticing an area you'd stopped feeling, feeling more awake, wanting to stretch, a movement suddenly freeing up. The nervous system isn't aiming for sedation — it's aiming for adaptive. My job is to give it less reason to work so hard.
How This Shapes My Sessions
My work combines channel-based Shiatsu with movement ideas from Sotai and close attention to how someone responds to touch. The theory comes from Chinese and East Asian medicine. The delivery — how slowly pressure arrives, when to use movement, when to just stay still — is shaped by the research above.
Sometimes the right move on a tight spot is direct pressure. Sometimes it's working somewhere else on the channel. Sometimes it's movement. Sometimes it's staying still long enough for the person under my hands to stop resisting.
The science of therapeutic touch is still young. But the old traditions and the new research seem to point the same way: touch isn't something done to tissue. It's information given to a living, sensing, adapting nervous system. How you apply pressure matters as much as where.
